A 54 year old 80 kg male with end stage
ischemic cardiomyopathy has just
received a cardiac transplant. The
donor, a 21 year old female weighing
55 kg, was on 8 mcg/kg/min dopamine,
had been hospitalized for 3 days
following a motor vehicle accident,
and declared brain dead for 8 hours
prior to cardiectomy.  The ischemic
time was 3 hours and 50 minutes and
the heart appeared "slightly small".
The recipient preoperative pulmonary
vascular resistance was 4.5 wood units
and the PA systolic pressure was 50.



The transplant is completed and follow-
ed by a period of cardiac resuscitation
and deairing. What is the first line
inotropic therapy?
